What are OCF Forms?

OCF Forms are also known as Auto Insurance Claims Forms.

These are a list of forms that Ontario automobile insurance companies require you to fill out in order for you to apply for accident benefits and/or make decisions about your claim for benefits such as: treatments, supportive care and expenses.

These forms and others that you may require are also available on FSCO’s – Financial Services Commission of Ontario’s website:  Download Forms

The forms are not always easy to understand.  Asking for assistance from a personal injury lawyer or your adjuster would be very helpful.

You must use the most up-to-date forms found on the FSCO website.

Forms must be filled out as “accurately’ as possible, otherwise it will delay the application of your claim which means it will take longer to receive your treatments or services.

Some forms must be filled out by your family doctor (or healthcare practitioner) and also your employer.

No money is given out to you until your insurance adjuster receives the completed and signed forms from this Application for Accident Benefits package, therefore it is essential that you apply for your benefits.

When a family physician fills out an OCF form on your behalf, he/she can also submit, at the same time, an invoice for completing the form/s.

Timeline to submit an Application for Accident Benefits (OCF-1):

The Application for Accident Benefits forms must be submitted to your insurer within 30 days from the day you receive the forms.
